Sign-up Forms

Please fill out the consent form for the programs you choose. You must fill out an individual form for every child you sign up. Once you fill out your form you will be sent to the Payment page. You may click on the programs you signed up for and pay using your credit card through PayPal, or send us a check by mail after calculating your total.
